Creekside Family Dental offers the Choice Care Membership Plan for patients and families without dental insurance. With one annual fee, members receive preventive care benefits and reduced rates on many dental services throughout the year.

Includes two dental cleanings, two oral exams, digital X-rays, oral cancer screening, and fluoride treatments.

The 12-month membership fee is paid once per year, helping make dental care more predictable and easier to plan.

Submit a simple online request with your preferred days, times, reason for visit, and contact information.
